Introducing FAPI PET: A New Frontier in Cancer Imaging

 

Parkway Radiology is proud to offer FAPI (Fibroblast Activation Protein Inhibitor) PET – a novel imaging technique that is transforming cancer detection and diagnosis. FAPI PET provides high-contrast images of a wide range of tumours by targeting cancer-associated fibroblasts offering enhanced sensitivity and specificity compared to traditional imaging methods. 

How does FAPI PET scans work? 

A radioactive tracer designed to bind specifically to FAP (a protein highly expressed by cancer-associated fibroblasts) is injected into the patient’s bloodstream.

The tracer flows throughout the body and accumulates in areas where FAP is highly expressed, especially in the tumour microenvironment. 

Benefits of FAPI PET scans

  • Improved Tumour Detection: FAPI PET is particularly effective in visualizing cancers that may be less visible on conventional FDG PET scans, such as gastric, pancreatic, liver, breast, and others.
  • Early Detection: FAPI scans may be more effective in detecting smaller tumours and early-stage cancers that may be missed out by other methods. 
  • Lower Background Uptake: It offers clearer tumour visualization with less physiological background activity, especially in the brain, liver, and bowel.
  • Faster Imaging with Minimal Preparation: Unlike conventional PET, FAPI PET scans does not require prolonged fasting or blood sugar monitoring, making the patient experience more convenient.
  • Evaluation of Tumour Microenvironment: FAPI PET may provide insights into tumour behavior, staging and monitoring treatment planning and response.
  • Lower radiation dose to the body due to rapid clearance by the kidneys.

What to Expect During a FAPI PET Scan

  1. The procedure is similar to other PET scans. A small amount of FAPI tracer is injected into the patient’s bloodstream.
  2. Patient will rest for about 30-50 minutes while the tracer circulates.
  3. The scan itself takes around 10 minutes, during which patient will lie still on a scanner bed as detailed images are captured.

How to Prepare

  • Minimal Fasting: You may be asked to avoid eating for 2–4 hours before the scan.
  • Medications: Most regular medications can be continued, but inform our team if you’re on any specific treatments.
  • Comfortable Clothing: Wear loose-fitting clothes and avoid metal (e.g., zippers, jewelry) where possible.

FAPI PET is typically recommended when other imaging methods provide limited clarity or when specific tumour types are suspected. Your referring doctor and our radiologists will evaluate if FAPI PET is the most appropriate imaging technique for your condition.
